Welcome to the Delaware Valley University Chosen Name Request Form
	The Chosen Name system allows the user to 'choose' a first name different from their birth name and have that chosen name displayed in various University IT systems. Upon successfully logining in, the user will be presented with a dialog box prompting for a chosen name and various checkboxes allowing the user to 'opt-in' or use the chosen name for various IT systems. Systems in which chosen name can be used are listed on the left side of the screen while systems where chosen name can not be used will appear on the right side of the screen.
